Joslyn Chevalier Director | Joslyn is the Director and Senior Support Coordinator of ACTIONABILITY . Since 2012, Joslyn has worked closely with countless people living with disabilities & their support networks. She continues to be humbled by the resilience and strength of those she works with, despite the immense challenges many are faced with daily. She understands that the individuals she works alongside are experts in their own life and she feels honoured to be given an opportunity to learn and find inspiration from her clients, gain unique insights, contribute to their growth and the pursuit of their goals in any way possible. Since the rollout of the NDIS in Western Sydney in 2016, Joslyn has worked alongside participants and their support teams providing Support Coordination services. Over the years, she has discovered and appreciated what "works" and definitely observed a few things that don't work so well where "the wheels fall off" The desire to do things a little bit differently and whole lot better led Joslyn to create ACTIONABILITY , where creative, genuine investment and individually tailored Support Coordination is the sole focus. Joslyn has ample experience navigating complex systems and pathways within the National Disability Insurance Scheme, NSW Health, Dept of Justice and mainstream community services. She has worked alongside many individuals to achieve their housing goals including ascertaining funding and facilitating successful transitions within Supported Independent Living (SIL) Specialist Disability Accommodation (SDA) private housing options, Aged Care (YPIRAC) as well as transition from Hospital into the community. | | Bella Luu Support Coordinator | Bella draws upon her strong background in individual support both in the disability and personal development space to deliver a highly personalised and tailored approach to her Support Coordination service. Currently active in the fields of Support Coordination, Mental health (Psychosocial Recovery Coach, direct support mentor, and counselling), Fitness (Pilates, Yoga, Group Fitness instruction and personal training), and Wellness (somatic movement therapy, meditation, sound therapy, trauma-informed Yoga); coupled with her bubbly, proactive and compassionate personality, Bella brings an energizing zest to her work. Bella endeavours to view a person's life holistically and build capacity within the individual (or family member) to better understand how the NDIS can work for them to support both their daily needs as well as short and longer-term goals. | | Sienna Brogan Social Worker, Speciliast Support Coordinator | As a qualified social worker, Sienna brings a diverse range of skills and experience to the ACTIONABILITY team and the individuals she supports. With over 4 years experience in the disability sector, including direct support, management of support teams, Support Coordination and counselling, Sienna is passionate about taking positive steps forward with those she works alongside. As a Level 2 and Level 3 Support Coordinator, Sienna has ample experience working with NDIS participants with complex needs, behaviours of concern, medical needs or other high risk circumstances. She is able to confidently addresses complex barriers that may affect the individuals she works with, including navigating and working within state and federal bodies such as the Dept of Justice and NSW Health. As a qualified Social Worker, Sienna provides mental health counselling services as well as general coaching and capacity building support. With her warm, open and non-judgemental nature, she treasures witnessing and supporting even the smallest "wins" in the lives of those she works with. | |
Sarah Meijnderts Social Worker, Specialist Support Coordinator | Sarah is a qualified Social Worker with ample experience working in the mental health sector. Sarah has experience working with both children and adults and is passionate about using a person-centred approach when supporting people. Sarah has been working as a Support Coordinator since 2018 and has a wide range of knowledge in the NDIS space. Sarah also has experience working as a disability support worker for many years and understands the importance of empowering people to make their own decisions. With a background in direct disability support, aged care, and independent living assistance, Hannah specializes in fostering independence and advocating for her clients. Her services include tailored Support Coordination designed to meet individual goals and needs. Hannah is a strong advocate for participant rights and supports clients in decision-making while working closely with the individual's family and allied health professionals to ensure a holistic approach to care. Her expertise extends to manual handling, PEG feeding, incontinence care and high physical and behaviour support needs, making her well-equipped to meet complex needs. With a flexible and adaptable approach, Hannah is dedicated to providing compassionate, person-centered support that promotes dignity and improves quality of life. | | Imelda Li Art Therapist | Imelda is a qualified and registered Art Therapist and Social Worker with ample experience working with individuals of all ages. She designs targeted art-therapy programs for children, families, and adults living with disabilities such as mental health diagnoses, autism, anxiety, PTSD, depression and addiction. Through art therapy, Imelda has helped many parents, adults, children and youth through grief, trauma and loss and provided support to those with significant behavioural challenges and a diverse range of diagnoses/conditions. Imelda is passionate about art therapy and is committed to supporting her clients in discovering their strengths through a creative, therapeutic process and guiding them through their journey to recovery, working towards their goals and developing their skills. She has worked in multiple settings, including government and non-government organisations. Art therapy can be an incredibly valuable healing and capacity building process and experience. | | Nikita Wood Support Coordinator | Nikita is passionate about making a genuine difference for the "one" (or each individual) as she recognises that in the NDIS world, some may feel treated like "just a number" or not really seen and heard. Nikita appreciates that every person is worthy and valuable with unique situations and needs. She strives to build and empower each individual she has the privilege of supporting with the aim to increase independence and confidence. Nikita believes in the adage "“If you give a man a fish, you feed him for a day. If you teach a man to fish, you feed him for a lifetime.” Likewise, Nikita strives to not only undertake the tasks required to implement an NDIS plan, but also increase and strengthen participant's and their support network's understanding, knowledge and confidence to navigate all things NDIS as independently as they wish to. | |
Natasha Morley Social Worker | Natasha is a qualified and experienced Social Worker with over ten years experience in the Disability and Community Services field. Natasha's experience ranges from working with young people in residential out-of-home care, School Leaver Employment Services / Transition to Work programs, direct support and, most recently, as a passionate and invested Social Worker, Natasha's education, experience and expertise make her an invaluable asset to our team.
